Plan (PDP) Forms
The following PDP and PDP change
forms are now required for all new WS license applications and changes to any
old PDPs (including those using the previous format/rules). Teachers
should no longer be submitting license applications using the rules 46 & 47
PDP forms. The forms below are required as of July 1, 2003.

customized formats.

Please note: All teachers applying for initial WS II licenses or
renewal of same should now follow the guidelines described under the new PDP
point chart for preparing the Professional Development Plans (PDP) that must
accompany any license requests. These documents are the equivalent to a
contract for the activities that must be successfully completed before an
expiring WS II license can be renewed.

WS License Application Guidelines
Note: Courses taken to
meet the Basic Skills test requirements require a grade of “B” or better,
and the pass/fail or credit/no credit will not apply. Acceptable courses are: From Ivy Tech: For Math: MATH 050, 070 or 080; for Reading: ENG 032,
for Language: ENG 025. Also: Any 100 level or above in
appropriate content areas can be taken at Ivy Tech or at any four-year institution. Equivalent courses from other institutions may
be considered for use as a possible course substitute.
Renewal PDP
Representatives
All WS II Proficient Practitioner license renewals require the use one of
two PDP representatives instead of one of the seven university
representatives used in the past. This include signatures on old,
completed PDPs. Note: First year teachers (only) applying for their
first level WS II Proficient Practitioner license that participated in the WS
I first year teacher training project will work with their online instructors
from the WS I Training Program (this course) when developing and seeking signatures for the
initial PDP.
All WS II Proficient Practitioner license renewals will be processed through the PDP representatives Mr. John Jandura or Mr. Gene
Pitts.
The university representatives who have processed PDPs in the past
will no longer be involved in the authorizations of completed or new PDPs
except for the first PDP after completing the OS I or WS I training project.

Both old and new PDPs should be sent
to the individuals above for final authorization. Both forms must
have the appropriate authorization lines signed by both the teacher and the
director (not other administrator) prior to mailing the documents to the PDP
representatives.
All supporting documentation
(verifying activities completed) as indicated by the PDP point chart must
be included with the information sent to the PDP representative. Only
listing the activities without official verifying documents will not be
sufficient.
Be sure to include the CTE director's
name, address, and employing school with the correspondence. All signed
documentation will be returned to the career and technical director (not
directly to the submitting teacher). Do
not send any PDPs to the former university representatives!

The following point chart
is
used for developing PDPs all license applications. Note that any TABE deficiency can no longer be counted as part of the 90 points. You must make up the TABE deficiency; but, it will not count on your PDP, effective with WS I licenses issued in 2009 (This includes ALL WS I teachers in the 2009-2010 WS I Training Program.).

Please note for the worksheet below
that there is a requirement for CPR certification for any new license that is
not yet included on the worksheet. If you are applying for an WS I or new
WS II license (and CPR was not required for the WS I when received),
then it will be necessary to also verify CPR certification.
