Blank forms and instructions for Downloading & Submitting New Study and Amendment Forms Please select the download for the highest classification. For example, if your animal use falls into both classifications B and D, complete the D form. Be sure to answer all questions in the spaces where the information is requested. Reviews cannot be completed if relevant sections of the application are not completed. Answers must be directly responsive to the question, concise, and formulated in lay terms as much as possible. Instructions • Class C Animals (Will not involve pain, distress, or use of pain-relieving drugs) • Class D Animals (Will involve pain, distress but pain/distress will be modified or minor) • Class E Animals (Will involve significant pain/distress.) • Teaching Study (Animals will be used only for teaching or demonstration purposes.) |

It is recommended that investigators use those portions of the Animal Care and Use Application as a guide in providing the information required when making changes to their approved studies. | Instructions for Submitting a New Study or Amendment Form | |||
| To submit your application and/or amendment, 1) upload it at: https://apps.iusm.iu.edu/iacuc/Admin.aspx and 2) send the original, with your signature, and one copy by campus mail to IACUC Office, I.U. School of Medicine, 541 Clinical Drive, CL 157 Indianapolis IN 46202. By uploading these forms you can be assured that you will meet the next deadline for submission (see submission deadlines above). A signed hard copy of each form will be required for final approval. | |||
