What changes do not require an Amendment Form?

• addition or deletion of project participants
• changing the location (building and room number(s)) for procedures
• changing study title
• adding different strains of animals
• changing the supplier
• changing the anesthesia (requires veterinary approval before implementation)
• minor changes to a breeding colony
• adding a procedure that will be performed on a euthanized animal
• changing the method of administration for a drug/agent (requires veterinary approval before implementation)
• changing the sex of the study animal

Notification and approval of the above changes is required before implementation; however, these requests may be acceptable via e-mail notification from the principal investigator. Allow at least 48 hours for review and approval of each request. The e-mail must be sent to SOMIACUC@iupui.edu and identify the studies involved by study number and clearly describe the change requested. For the addition or deletion of project participants, it must also include their name and also their e-mail address if available. Newly-added participants may NOT work with animals until documentation of training is approved.
