Principal Investigator Responsibility for Documents

It is important that you KEEP A COPY of every document that you submit. For audit purposes, these documents must be kept for at least 3years after terminating the study. The IACUC Office will NOT be responsible for duplicating study information.

Note: Continuing review forms are generated from the IACUC office for each study. If you wish to close a study prior to the next review, request a form by sending an e-mail to somiacuc@iupui.edu
