Animal Care and Use Application and Amendment Forms
Regulations require that an Institutional Animal Care and Use Committee
(IACUC) approve all procedures involving the use of animals prior to
their initiation. To comply with these regulations, an Indiana University
School of Medicine (IUSM) Animal Care and Use Application Form must be completed.
The application form fulfills two purposes: 1) It allows the local IACUC
to perform a federally mandated review of goals, protocols and procedures
used in the proposed study to insure the humane treatment of animal
subjects. 2) It serves as a record of the study protocols that can be
reviewed by responsible outside agencies, such as AAALAC, USDA, FDA,
or PHS. Therefore the application needs to be written clearly and concisely
so that it is understandable by a broad audience of technical and non-technical
reviewers. It is also advised that investigators carefully review each
question and assure that the responses are complete. Incomplete narratives
will delay approval of the proposed activities.
