Please note that international students with University Fellowships (semester stipend of $3077 or greater) or assistantships (Student Academic Appointments of.375 FTE or higher) are covered under a separate health insurance plan. Insurance premiums are billed to and paid directly by the academic department, and the coverage is similar but different from the plan described on this page. All enrolled international students in F or J visa status are automatically enrolled in IUPUI's mandatory health insurance program for international students. International students in other non-immigrant visa categories are also eligible, but are not automatically enrolled. These students must contact the Office of International Affairs for assistance.
Students who are U.S. citizens or Permanent Residents are not eligible for the International Student Insurance Plan.
All enrolled international students in F-1 or J-1 status will automatically be enrolled in the mandatory insurance plan unless they have been approved for a waiver. Billing occurs in September for Fall premiums and in January for Spring/Summer premiums. The charges will appear directly on your Bursar account.
After billing occurs, you will receive a card in the mail sent to your home address that you have entered into Onestart. You are eligible for the health insurance benefits even if you do not have the card. A temporary card may be downloaded here. This card can be used until your official card arrives in the mail.

Eligible dependents are the spouse/same-sex partner (residing with the insured student) and unmarried children under the age of 24. If you have dependents, you will need to enroll them by completing the dependent enrollment form, which can be mailed directly to Aetna with your payment. Dependent enrollment is also available online at www.aetnastudenthealth.com. Click on “Find Your School” and enter 812849 as your Policy Number. Unlike your own coverage, it cannot be billed to your Bursar bill. The completed Dependent Enrollment Form and premium should be submitted directly to Aetna Student Health, Inc, One Charles Park, Cambridge, MA 02142-1254.
Please remember that all international students are required to maintain medical insurance coverage for their dependents. For Fall 2009, Dependents must be enrolled with Aetna by September 15, 2009 or are not eligible for coverage until the next enrollment period.

If you already have other health insurance coverage you may apply for a waiver from the mandatory plan. Not all insurance plans will qualify you for a waiver.
The minimum coverage requirements for waiver approval are as follows:
To qualify for a waiver, students must have qualifying insurance for the entire coverage period. Students applying for Fall waivers, must have coverage for at least August 15th through December 31st. Students applying for Spring/Summer waivers, must have coverage for at least January 1st though August 14th.
All waivers are submitted through the iStart system. Regardless of the length of your insurance coverage, a new waiver must be submitted each semester.
The waiver deadline is always 10 days after the start of classes each semester. The Fall 2009 deadline is September 4th. Waivers will not be accepted after this date.
Some categories of students qualify for automatic waivers from the International Student Insurance Plan. These students are not required to complete the waiver request through iStart. Your insurance plan includes a $500 deductible. This means that the first $500 of your medical care each year (in addition to your premium payment) is paid by you. After your deductible has been met, Aetna will pay all covered medical costs. Please read the brochure carefully to understand the services that the plan covers. Remember, no insurance plan covers 100% of all medical costs. In addition to a deductible, the plan requires that you pay a $15 copay for each doctor's office visit.
The plan waives the $500 deductible and $15 copay for treatment you obtain at the Student Health Service (SHS), located on the first floor of Coleman Hall (beside Ball Residence Hall). The SHS provides basic non-emergency medical services; you may schedule an appointment by calling 274-8214. Services received at the Student Health Center will be paid upfront by the patient at the time of treatment. However, you may then submit your bill to Aetna Student Health for reimbursement.
If you need to seek medical treatment outside of the Student Health Service, you will need to seek a doctor within the Aetna network. The insurance program uses a “Preferred Providers Network.” When you visit a physician that is part of the network, all expenses covered by your health insurance (except for exceptions listed in the brochure) are paid by the insurance company at 100%. If you use a physician or hospital outside of the network, the insurance company will pay only 50% of “reasonable” costs.
The Aetna Insurance network is extensive and includes all hospitals and clinics within the IU Medical Center and Clarian Hospital networks, including University Hospital, Riley Hospital for Children; Wishard Memorial Hospital, Methodist Hospital of Indiana, and Methodist Medical Plazas located in various parts of the city. In addition, it includes the Community Hospitals.
