Federal SMART Grant Verification Form

The Spring 2009 Federal SMART Grant Coursework Verification/ Major Academic Progression Form is available for students and advisors to complete to satisfy the requirements for a Federal SMART Grant review. A student can receive a SMART Grant only if they are enrolled in at least one course that meets the specific requirements of the student's SMART Grant-eligible major. General education courses or electives that satisfy general degree requirements for a program must be accompanied by at least one course specific to and required for the SMART Grant-eligible major. If this requirement is not met, students will not be eligible for the SMART Grant for the upcoming semester. Due to this requirement it is mandatory that the student and advisor complete the Federal SMART Grant Coursework Verification/ Major Academic Progression Form.

*Please note that a new form is required for each semester, including summer.
