IUPUI Procedures


Steps for Purdue Undergraduate course approval

  1. Department initiates a Purdue request for addition, expiration, or revision of a course (Form 40).  Two copies of the signed Form 40 will be returned to the department contact for their records once the Purdue registrar signs the Form 40.
  2. Once the Form 40 is processed and approved, Department initiates IU course change or new course form, making sure the academic subject code and course number are available for use. [A Form 40 is initiated as well (delete this last line but use the links) Also, use links for course change and new course forms]
  3. The Department Chair signs form or the School Curriculum Committee may review, approve and sign form

  5. Dean approves and signs the course form.
  6. Forms are forwarded to the Office of the Registrar, Attn:  Thomas A May, Cavanaugh Hall 141.   
  7. Here the Purdue undergraduate Form 40 (with 5 copies if the course is new) and a copy of the IU course form is forwarded to the Registrar’s office at West Lafayette. Two copies of the signed Form 40 will be returned to the department contact for their records once the Purdue registrar signs the Form 40.
  8. The IU course form goes on remonstrance when the next remonstrance period comes up, and if no one has a remonstrance is ready to go onto Bloomington once the Form 40 has been returned to the IUPUI campus with the appropriate signatures
  9. Once both the Form 40 and IU course form have gone through the appropriate channels, the Dean of Faculties signs the IU course form
  10. The IU course form is then sent to Bloomington's Student Enrollment Services for coding and entry into the course catalog.
  11. Two copies of the IU course form will be returned to the department who initiated the course form.

Steps for Purdue Graduate course approval

  1. Department initiates course change or new course form, making sure to follow the  IUPUI Graduate School requirements as well as the Purdue Graduate School course guidelines (see section I). A Form 40 is initiated for both course change and new courses. There is a supporting document for new graduate courses (Appendix B) in the Purdue Graduate School course guidelines.
  2. The Department Chair signs the forms, or the School Curriculum Committee can review, approve and sign the forms.
  3. Dean approves and signs forms.
  4. The IU course form with the appropriate materials specified by the IUPUI Graduate School can be sent to the attention of Beth Young, UN 207 which she will forward on to the Graduate School's Graduate Affairs Committee for approval.
  5. The Purdue School on the IUPUI campus is responsible for contacting and getting the Form 40 signed by the appropriate chair(s) and dean(s) at other campuses if necessary. Once the appropriate signatures have been obtained the Form 40 can then be sent to IUPUI Graduate Office with a copy of the required materials specified in the Purdue Graduate School's Policies and Procedures manual under section I.
  6. The Form 40 regardless of whether it comes from IUPUI Graduate Office or is sent directly, goes to the Graduate Office at West Lafayette for approval by the Purdue Graduate Council. Once the Purdue registrar signs the Form 40 three copies are sent to the IUPUI Graduate Office. The IUPUI Graduate Office then sends two of the three copies of the signed Form 40 back to the department for their records.
  7. After the IUPUI Graduate Affair's Committee signs the IU course form, and if the Form 40 has been signed by the Purdue Registrar, the IUPUI Graduate Office will send the IU graduate course form to Bloomington for coding by Student Enrollment Services.
  8. After the course has been coded, Student Enrollment Services sends the blue, pink, yellow and sometimes white copies of the coded course form to the IUPUI Graduate Office. The IUPUI Graduate Office then sends the pink and yellow forms to the department contact we have listed on our list of department contacts.
