DEADLINE EXTENSION: JOSEPH T. TAYLOR AWARDS FOR EXCELLENCE IN DIVERSITY 2003

The deadline for submitting nominations and applications for the third annual IUPUI Excellence in Diversity Awards has been extended until Jan. 17, 2003.

The IUPUI Chancellor's Diversity Cabinet is seeking nominations and applications from students, staff, faculty and community members for the awards, presented in honor of Dr. Joseph T. Taylor, the first dean of the Indiana University School of Liberal Arts at IUPUI. The chancellor will present the awards at the Joseph T. Taylor Symposium on Feb. 14, 2003. Awards will honor IUPUI academic and support programs, events, policies and activities for exemplary achievements in three categories:

Recruitment and Retention of Underrepresented Groups
Increased recruitment and/or retention of faculty, students and staff from underrepresented groups.

Diverse Campus Climate
Enhanced campus climate for diversity through social, cultural or developmental programs or events.

Innovative Academic Activities
Advanced campus engagement in issues related to race, class or gender through innovative curriculum, research, programs or events.

Health Care Plan ID Card Update

In early January, the university will mail employees confirmation of the health care plan changes made during open enrollment. The confirmation notice can be used as a temporary ID card until an official ID card is received from the health plan. Health plan administrators will not mail ID cards before the end of January. With the recent conversion to the new HRMS system, data entry and electronic transmission of the enrollment information to the health plan administrators will occur in late December. Health plans will mail ID cards within two to three weeks after receiving enrollment information

Anthem will issue ID cards only to employees who are new to the plan (PPO $900, PPO-Plus, or Blue Preferred) or who have changed PCP's (Blue Preferred).

M-Plan will issue new cards to all members. M-Plan is converting to a new system, and members also will receive a new ID number.

CIGNA will not issue new ID cards. A CIGNA ID card can be obtained at http://www.indiana.edu/~uhrs/benefits/cignadentalcard.pdf or by sending an e-mail request to benefits@iupui.edu.

Prior to receiving ID cards, if employees encounter problems in scheduling urgent appointments, the health plan should be contacted first at the numbers listed below. If the health plan is unable to verify enrollment, the benefits office should be contacted at 274-4596 or benefits@iupui.edu. If a pharmacy is unable to process a prescription for the plan co-pay, the employee will need to pay for the prescription and submit the receipt to the plan for reimbursement.
