Step-by-step Instructions for Completing the Application Form
 
Fill out both sides of the enclosed application by typing or printing in black or dark blue ink. Please use uppercase (capital) letters. Write in “Not Relevant” or “Unknown” in those spaces that do not pertain to your situation. Note that all dates should be indicated in the western calendar. Please try to use the American convention of Month/Day/Year to avoid confusion. Sign and date the application on the backside after reading the “Certification of Truth” statement.

Legal Name. Write your name exactly as it appears in your passport. Your family name (last name or surname) should come first. Because all applications are filed alphabetically by family name, it is important that you use the same English spelling of your name on all correspondence with IUPUI. Please be sure to also write your name at the top of page two of the application. 

Social Security Number (SSN). If you do not have a U.S. Social Security number, the Office of International Affairs will assign a University ID number when it receives your application. The University ID number will be used to identify your application. Please refer to it on all correspondence with IUPUI. Canadian citizens should not provide their social insurance number.

Variations of your Name. List all variations of your name that will appear on your records. This will help the Office of International Affairs match your application with test scores, transcripts, and financial documents that arrive separately. Please use the additional space on the back of the form, if you need it.

Country of Citizenship. Please be sure to enter the name of a country, rather than a territory or region. This is usually the country that will issue your passport.

Immigration Status. If you are not a citizen of the U.S. and you are currently in this country, please indicate the visa type noted on your I-94 (Arrival/Departure Record) in the space provided. If you have never been to the U.S., you can leave this section blank.

Ethnicity. If you are a U.S. citizen, or if you are have permanent resident, refugee, or political asylum status in the U.S., you have the option of identifying your ethnicity in the space provided. Otherwise, leave this blank.

Dates Lived in Indiana. If you have permanent resident, refugee, or political asylum status in the U.S., and think you may qualify for Indiana residence for tuition purposes, please indicate the time that you have lived in Indiana.

ADDRESS & CONTACT INFORMATION
Present Address. This is the address we will use to correspond with you. Please write down your address, as it should appear on an envelope. If you have a telephone and/or fax number, where you can be reached, please write them down in the spaces provided. If you want us to correspond with you via E-mail, please provide your E-mail address in the space provided.

Permanent Home Address. If your permanent home address is different from the Present Address, please write it down, as it should appear on an envelope. (If they are the same, you may leave this blank.) If you plan to return to this address before attending IUPUI, please tell us on what date you expect to move back to this address. (We will only correspond with you at this address, if you tell us when you will return to it.) If your telephone and/or fax numbers are different at this address, please provide them. Similarly, if you have a different E-mail account, please write this down. If you are on a F1 or J1 visa, you must provide your foreign address.

Name a person to contact in case of emergency (optional): 
In case we are unable to reach you, please give us the name, address, and telephone number of someone who can get mail to you. If you want this person to act on your behalf in applying for admission, please mark the box yes. If there is someone else whom you want to act on your behalf, please give us that person’s name, address, phone number, E-mail address, and fax number, if available, on a separate piece of paper. The paper should also state formally, “I hereby authorize… to act on my behalf in applying for admission to IUPUI. You may address all messages about my application to this person as if you were communicating with me directly.”

RESIDENCE FOR FEE PURPOSES
In most cases, to qualify for residency status, students must (1) host U.S. Citizenship, lawful permanent resident, refugee, or asylee status and (2) have lived in Indiana for 12 months prior to enrollment. However, several exceptions exist. Please check our Residency Information to see if you may qualify for consideration as a resident.
  1. Please tell us how long you have lived in Indiana If you have permanent resident, refugee, or political asylum status in the U.S., and think you may qualify for Indiana residence for tuition purposes, please indicate the time that you have lived in Indiana.
  2. Upon whom do you depend? Use this space to indicate whether you are financially independent or if you can be claimed as a dependent on a parent’s, spouse’s, or legal guardian’s Federal Income Tax Return.
  3. If you depend upon someone else: If you are financially independent, residence for fee purposes will be based on your information. If you are a dependent, residence is based on the eligibility of the person or persons who claim you as a dependent. Be sure to give us the dates that person has lived in Indiana. The Office of International Affairs will make an initial residency decision based on the information that you provide here. The university assesses tuition based on whether or not the student is considered a resident of the State of Indiana for Fee-Payment purposes. The rules for determining residency are established by the Trustees of the University. The initial determination is made at time of admission. If the student is found a non-resident and wishes to appeal the decision, she or he should contact the Office of the Registrar for an Application for Classification as a Resident Student. IUPUI STUDY PLANS
What will be your major subject of study? In the space provided, write down the name of the subject you want to major in at IUPUI. The list of graduate and professional majors and degrees begins on page 19, below.

Planned degree. Again the list of academic majors and degrees is located here. Please read this carefully. Not all graduate programs offer both master’s and doctoral degrees. If you plan to earn only a master’s degree at IUPUI, mark the box next to “Master’s”. If you plan to earn both a master’s and a Ph.D. degree or only a Ph.D. degree at IUPUI, mark only the box next to “Ph.D.” If you plan to pursue a professional degree program at IUPUI, mark the box next to “Other” and write down the name of the degree program that you want admission to.

When do you want to begin? Please tell us which term you want to begin, by marking the box next to the appropriate month and filling in the last two digits of the year. (See Enrollment Periods) Be sure to check with the graduate studies committee or admissions committee in the p rogram that you want to begin. Most graduate degree programs begin in (Fall Semester). Some programs can be started either in Fall Semester or January (Spring Semester). A few professional programs begin in the summer.

PREVIOUS APPLICATION INFORMATION
Are you currently attending the ELS Language Center in Indianapolis? If you are currently, or were within the last 6 months, enrolled at the ELS Language Center in Indianapolis, please mark the box yes and write down your ID number. 

Have you applied for admission to IUPUI before? Please mark the appropriate box. If you tell us that you have applied before, please let us know when and to which program you wanted admission previously.

PREVIOUS EDUCATION
Please fill in the information requested in the space provided. Please do not translate educational terms, especially degrees, into English. Terms, such as bachiller, licentiate, diplom, zeugnis, or brevet, should be written in the language of the educational system. Be sure to list all colleges or institutions of higher (post-secondary) education that you have attended. If you are applying during the final year of college, indicate on the application the name of the degree you expect to receive upon completion and when you expect to graduate.
