WORK-SITE SAFETY EVALUATIONS

IU Occupational Health Service physicians are concerned with the work environment of the IUPUI faculty and staff.  Our physicians make work-site safety evaluations, ergonomic evaluations, and referrals to specialists as needed for the safety of all IUPUI employees.

If you feel your work-site should be evaluated, complete an incident report (obtain from your supervisor).  Bring the completed incident report form to IU Occupational Health Services for an appointment with a physician.  The physician will determine whether or not a work-site evaluation is necessary.
