WORK-RELATED INJURIES OR ILLNESSES

If you sustain an injury while at work, notify your supervisor immediately.  An Indiana University incident report must be filled out and must include your supervisor's signature prior to being seen at IU Occupational Health Services (your supervisor should have one).  Please pay attention to the following areas on the form:

Account number

Payroll clerk name and telephone number

Wage rate

Employment date with IUPUI

Employment date at this job

Your employment dates can be obtained from your supervisor or your payroll clerk.  A detailed description of the incident must be included along with names and telephone numbers of any witnesses.  Once seen by the IU Occupational Health Services staff, you will receive a "back to work" form which will include any work restrictions.  A copy will be given to you for your records as well as a second copy that you will need to provide to your supervisor immediately following your visit to IUOHS.  This form will also have your return appointment time on it.  It is very important to come back to IUOHS for all your follow-up appointments.

If you are referred to a different physician, you will need to return to the IU Occupational Health Services clinic after your appointment.  The IUOHS staff will need to see any paperwork that the referring physician provided to you.

If an injury occurs after hours or on weekends do one of the following, depending on the severity of the injury.  For injuries that can wait, come to the IU Occupational Health Services clinic on the morning of the next business day or for injuries that can not wait to be seen go to the University Hospital Emergency Room.  Depending on the availability of ambulance services at the time of the injury, it may be necessary to go to Wishard Memorial Hospital.

Please make sure you come to IU Occupational Health Services for any follow-up visits.  The staff working at the office of Risk Management emphasizes that if you do not return for the follow-up appointments, you may lose your worker's compensation benefits.
